Acknowledgement of Rules

All participants must fill out this form every year in order to participate.

Previous Athletic Participation Form

This form is required if you competed in athletics after entering 8th grade at a school other than East Texas Charter School. If we have one on file then we do not need you to fill it out.

Physical Evaluation

This form is required every other year.

Concussion Awareness

This form is required for all athletes.
